 I had this pop up back when I was using squid beta 11 in a situation
 the same as yours...

 I remeber I made two changes... but I can't remember the second :)

 The first is to proto.c, change the line

 if(!protoData->inside_firewall) to
 if(!protoData->inside_filewall && request->protocol != PROTO_FTP)

 and the second one shouldn't be too hard to find i guess.. if you
 get stuck scream and I'll go through my mail archive. Alternatively
 I think there is an archive for squid-users somewhere.. and that should
 have my patches detailed in it..
