I've never found what's being going on in the logs, simply due to a lack of
time, but I've noticed I get a lot of ERR_DNS_FAIL in the log for good
addresses, e.g. www.microsoft.com occasionally! This is Squid 1.1.10.
Perhaps this is related.

I'll have a look to see what the access log says when this happens.
