Squid sending old page from Cache...

Hi,

How do I make Squid to fetch the updated objects from the source and send
to the client if the object in the cache is out-dated??

I tried setting the default refresh pattern :

   refresh_pattern . 0 20% 4320

OR with the following :
   refresh_pattern ^http:// 0 0% 0

It doesn't work. It still sends the old object from the cache. I am running
Squid v1.1.10 with DEC Alpha OSF 4.0B.
