Very likely - as that stores the mappings of url to swap (disk) file,
across restarts, if you had a shutdown that resulted in the disk being
full, or the disk had filled and then squid had carried on storing and
reusing swap file slots then things could well be hopelessly wrong.
(I thought there was a file size check - make sure you're running
recent versions)
