quick_abort and refresh_pattern

I have set the

quick_abort 5 50 100

I used

cp /dev/null access.log
tail -f access.log

then went to another pc .
typed

http://www.cnn.com in netscape.

and when it says transferring data I press stop.

and waited for two minutes

and then went to another pc and typed
http://www.cnn.com

but the access.log says

TCP_MISS

why is it???

and one more

could any body lemme know the exactly
refresh_pattern min % max

I need illustration of above in terms of IMS and
could anybody specifically definee above things
