How to pick Cache Size

I am wondering if anyone can tell me how to choose a cache size that will
maximize hit rate while not going wild with RAM and disk space. We are an
Internet Service Provider. Are there any good guidelines for disk cache
per dialup line or per user.

Our current setup is only 750 MB disk cache and squid (1.1.17) is running
very well in 8 MB of RAM. The system is a 686-166 with 64 MB of RAM and
also acts as a mail server - disks are IDE. We definitely would like to
add more cache but I see many people running huge caches 4-12 GB and would
like some guidelines as to where it would become redundant to add more
cache.
