Here's one possible explanation:

Some grouping of memory structures exceeded the size of cache_mem. On my system,
requesting the cache-objects listing will do this, and definitely cause squid to
crash, by spiralling out-of-control, consuming every available byte of RAM and
swap (an extra 130MB, in toto), and then crashing due to an allocation failure.
When this happens, my logs look pretty much like your logs below.

Does it happen when the cache-manager interface is used, or at other times?
There _are_ other things that can cause cache_mem to be exceeded.
