This is the exact configuration im using and it is working for me .. Me
configuration is :

For ipfilter - just this entry for ipnat

rdr ba0 0.0.0.0/0 80 -> proxy. interface 3128

For Squid:

http_port 3128
httpd_accel proxy.interface 80
httpd_accel_with_proxy_on
httpd_accel_uses_host_header on
