Then you have already uncommented USE_PROXY_AUTH. The default is
AUTH_OPT = # -DUSE_PROXY_AUTH=1

You should consider upgrading to Squid 2.x (which included proxy
authentication by default). The development of Squid 1.x has been
stopped, and no further fixes for that generation of Squid will be
provided.
