This is usually that Squid failed to connect to the site. There is a bug
that makes Squid forget to show the error message sent by the FTP server
when there is to many users connected to the FTP site.

This is partially fixed by 2.1.PATCH1. See
http://hem.passagen.se/hno/squid/ for a complete patch (probably applies
to all Squid 2 releases).
