Your "refresh_pattern" and "no-cache" ACL (if set) lines are probably more
relevant, if you can send those along. There doesn't seem to be any obvious
reason, unless someone knows of some obvious bug in PRE1, but I doubt PRE1
would have got out with such an obvious problem. I hope :-).

You could also try looking at your store.log for more info. I assume you've
already looked at your cache.log.
