That is quite normal, and has been the way Squid uses the cache
directories since 1.1.something.
To use more than level 1 directory you need a cache of at least the size
store_avg_object_size*L2*L2/2, and you also need to have this amount of
traffic (L2*L2/2 objects) sent throught your Squid before the next top
level directory is used.
