All authentication and password handling is outside Squid. To implement
password changing you would implement a CGI program changing the
password in the system Squid is interfaced to (NCSA password files is
only one of the endless possibilities).

Yes, at least partially, by defining ACLs that matches the different
user groups you can say that a certain group of users is limited (see
delay_pools).
