This message is only seen if you have enabled debug logging of level 2
(or higher) in section 14 (ipcache).

It is not harmful, and should be expected every now and then in normal
operation.

It says that Squid has detected problems with the IP address (failed to
connect) and is marking it bad to avoid future uses of that IP address
if there is other IP addresses for the same domainname that is working
fine (useful on clustered web servers where one or more of the servers
is dead, no effect on single address servers).
