SWAPOUT .. OK But TCP_MISS 80%

Hi guys
     I am using Squid 1.1.22, there is strange thing happening. Store.log
shows 90% SWAPOUT but when user accesses again the swapout url "access.log"
shows TCP_MISS. I have checked squid.conf many time and comment the
no-cache, cache-stop-list etc directives also.. even I have allowed cgi and
?. pls help me.
