PATCH: change cache digest settings via squid.conf

is a patch against Squid 2.2 STABLE3 which extrudes a number of
Squid's previously internal and hard coded magic numbers for cache
digest generation out into squid.conf.

It also lets you control whether Squid generates cache digests, so you
can compile Squid with cache digest support enabled - without your
cache being forced to generate its digest.
