Hi,

1. it is not a *nice* solution to use IP addresses for access control.

2. it is X-Forwarded-For (and *not* X-Forwarded-From) header that is used
by squid, if the forwarded_for option is turned on.

3. I don't think that Apache supports access lists based on either a
header or the client's IP address. You may want to write a CGI script
which can authenticate your clients coming directly or through a cache
server to Apache.
