Just set these correctly and the cachemgr will running.

acl proxy-admin src your.ip.address/255.255.255.255
acl manager proto cache_object
http_access allow manager proxy-admin
http_access allow proxy-admin
http_access allow manager
cachemgr_passwd password-for-cachemgr info stats/objects server_list
storedir store_digest 5min

I'm using the above settings and it works for me :-)
