Squid 2.2.x is even worse than 2.1.x and earlier.

Good news is that is now is configurable. If you have non-ICP parents to
Squid 2.2.X then set "prefer_direct off" (or apply a peering patch from
my patch page to get non-ICP peerings to work more like ICP peerings
with respect to what kind of requests are sent to parents).
