Always TCP_MISS

All,

We are now testing Squid as a reverse proxy with the following configs
and found that all log records in access.log are ended with TCP_MISS. I
have tested to reload the page a second time by clearing browser caches
but the reverse proxy does not really work. Please help.

PS: I am using squid-2.3.STABLE2-src.tar.gz.
