this. check your configuration file in %squid_dir%/etc/squid.conf

first check your http port
in squid.conf:
if you don't use default http_port 3128, chang # http_port 3128 => http_port
your_port

next, check your icp port
the default icp port is #icp_port 3130

last check your access right
search "INSERT YOUR OWN RULE(S) HERE TO ... "
default : http_access deny all
look at squid FAQ from squid home page.

and you do not need to set your IP address in squid.conf
