For "real-time" graphs, MRTG is a good tool.

NONE in the hierarchy tag means Squid did not need to contact someone
else to solve the request.

Non exactly, but there is a good tool for checking if a object can be
cached or not, with descriptive comments. I never seem to remember the
URL but it has been posted on squid-users numerous times (last time
yesterday I think..). Should also be a link from www.squid-cache.org
somewhere.
