I've seen this behavior with Squid 2.2STABLE5 on FreeBSD. It is caused
by some weird interaction between the code that re-opens the log file,
and the openlog() function. If you start squid without the -s option the
problem will go away.

I've been tearing my hair out over this one, as I fail to see where in
the code the offending action with file descriptor 3 occurs...

You might give it a try -- you may have a different problem than I had
but it sounded too familiar...
