Probably the request was forwarded to the cache peer.

See the always_direct tag for forcing Squid to go direct to certain
locations, ignoring any cache_peer settings.

One example:

acl infotrac-custom dstdomain .infotrac-custom.com
always_direct allow infotrac-custom
