[SQU] NTLM Branch(es)

For the daring that use the auth_rewrite and NTLM branches
from sourceforge CVS, I recommend updating.
A trivial bug prevented the NTLM authentication code to
properly reuse the NT challenge hashes, and this resulted
in worse performance and increased Domain Controller load.

This is to say, update :-)

Henrik, Duane, or whoever else can, please merge the
changes from auth_rewrite in.
