example from mind:

acl ALLOWED_PROTO proto http
acl ALLOWED_HOST dstdomain host.domain
acl ALLOWED_PORT port 1024
acl all src 0.0.0.0/0.0.0.0

http_access allow ALLOWED_PROTO ALLOWED_HOST ALLOWED_PORT
http_access deny all

i think, this is covered in detail in the squid configuration guide.
