It's a dumb question but I have to ask - have you restarted/reconfigured
squid since squid.conf was changed to what you showed us?
What you have sent should allow:
unrestricted access to hotmail
password required for everywhere else
If that's not happening I don't know why. Maybe you need to turn some
debugging on. Not sure what values though. A quick look at the code
reveals 28,9 might be useful for debug_options, eg
debug_options ALL,1 28,9
