Hi,

Things to check:

1) permissions for all path components for squid user to access
   /path/squid_rad_auth

2) if you have packet filtering (ipchains, iptables, ...) do they allow
   localhost access

3) id the process (/path/squid_rad_auth) actually starting?

4) check the squid logs
