[squid-users] Blocking specified users and sites

Hey ppl.
This question was asked before but i have recieved no reply.
How do I set up squid so it blocks access to particular sites (i.e. I have a
list of sites that I dont want users to have access to)
I run ldap_auth to Novell servers, and it has some problems, but the
problems are on the squid side, not novell. Problem is that somtime some
users cant get authenticated, i.e thay have entered thaie correct username
and password, ldap_auth returned "OK", yet squid just pops up authentication
dialog again and after 3 trys it finally denies user access. How would I go
about fixing that (i.e. i want all rightfull users to be able to access
internet, and I do have a list of users who I want to deny cache access).
