Which Squid version is this?

  squid -v

This type of shutdown is normally only seen when Squid is shut down by
someone killing the process. Have seen very few reports of unexplained
shutdowns, and in most cases it has later been found to be another
administrator doing so without telling or a misbehaving cron job going
berserk..
