If the cache_peer works like an "http_access" you can just write the
conditions one after another to have them ANDed.

When you need an OR you write:
acl condition1 ...
acl condition2 ...
http_access allow condition1
http_access allow condition2

When you need an AND you write:
acl condition1 ...
acl condition2 ...
http_access condition1 condition2
