url_regex ^equifax can never ever be true. There is no URLs beginning
with the letters equifax, as all URLs begin with the protocol name
(http://, ftp://, gopher://...

Are you sure url_regex is the ACL type you are looking for? From the
description of your problems it looks like what you are actually
looking for is dstdomain or dstdomain_regex.
