Hello Andrew,

When does this happen? Just after starting Squid?

Did you also check the cache.log file instead of the syslog?
What you can do I increase the debug value, this will give you a lot of
more information, and maybe you can send us those results.
For example: debug_options ALL,8 (= a LOT of messages , but it will give
you a clue :)
