There is no program accepting new connections on the log pipe you
created.

If having a program reading from log pipe you must make sure the program
automatically reopens the pipe when Squid closes the log file (happens
at restart or on squid -k rotate).

But I still maintain my previous recommendation of not using a pipe and
instead monitoring the log file created by Squid.
