 Don't know directly, but you may want to include the
 squid version being used and which platform/os/version
 you are using squid on.

 Suggestion : - check your system logs, and look for possible
 hw problems to exclude them first, watchout for disk problems.

 Under Linux for instance,check :
 
 % dmesg
 
 and /var/log/messages.
