Can be many reasons. Usually this is due to malfunctions in network
equipment at the requested web site or bugs in the web server.

A quite common reason if you run Linux or another operating system with
a modern TCP/IP implementation is because the site has a broken firewall
which does not understand that TCP has evolved since the original
specification.. See the Squid FAQ entry on Linux for instructions how to
downgrade the Linux TCP/IP implementation to work around such broken
firewalls (ECN).

There also exists a few broken load balancers on the marked which really
is not suited for load balancing HTTP traffic, and these may fail badly
on requests being sent via Squid, if the user is using other browsers
than the load balancer have been tested with, or simply if the user is
behind a network link with a limited MTU.

In all cases complaining to the site operators of the unreachable sites
is a good idea. Site operators generally want to know if there is a
problem to use their site.
