Try with a higher nounce max count (and duration).. 2 is very low. In
a proxy setup very long nounce lifetimes are generally acceptable.

Also try not disabling check_nonce_count.

If you like us to look into more detail what is going on then enable
log_mime_hdrs, then open a Squid bug report and attach the output for
one client and the login+password used. This was we can analyze the
authentication sequence and determine if the problem is in Squid or
Mozilla (but my bet is Mozilla).
