From the NTLM hash passed by the client. How the client gets the info
it needs for the hash is up to the client.

With Internet Explorer, yes - it uses the username and password used
to login to Windows. Recent versions of Mozilla also support NTLM
(Windows-only), but Mozilla still prompts the user for a username and
password.
