Neither is better - each is best suited for certain needs.
NCSA is best used when you don't have an existing authentication structure
and just need a basic password file to authenticate against.
PAM is useful if you want to use your system's native authentication.
If you want to authenticate just Squid (not your entire system) against an
authentication backend (such as LDAP or MS Active Directory), you should
use a helper designed for that.
