There is a couple of bugs/misfeatures causing this, most notably if you
run "squid -k reconfigure".

It should not happen in normal usage however.

No, Squid does not have any hierarchical delay pools allowing for
bandwidth lending.

In general yes but it depends a little on how this "dynamic bandwidth
charge" works as there will be short bursts of higher traffic while the
TCP receive window is filled.
