This is caused by the site not giving correct expiry information on that
"temporary out of service" page, making the cache think it is out of
service much longer than it was...

Usually it is sufficient to just press the reload button to resolve this
situation, but if you are transparently intercepting port 80 then this
won't work with MSIE browsers.. (not a bug, just a fact as most versions
of MSIE correctly assumes there is must be no caching proxy inbetween
itself and the origin site if not configured to use one..).

Note: If the clocks are or was off on either the web server or your cache
then a forced reload may be needed (Shift/Ctrl+Reload).
