From where are these two log lines?

A 403 is someone denying access.

The first line indicates access denied by "xxx.xxx.xxx.xxx".

The second line which is apparently from a different server indicates this
other request was dened by this server.

Usually there is no problem, other than that NTLM/Negotiate authentication
can not be used.
