Out of curiosity, if i leave out the cache_dir line, what settings would squid use?

The configuration is pretty simple. '16 256' is a bit small for a 120 gig
cache. You might want to up that to at least 64 256.

Is the server doing anything else besides Squid? Whats 'vmstat 1' show the
CPU load is like? What about memory and disk usage?

Other than that, i'd try it out without the authentication ACLs to start with.
I'd check your nameserver configuration and make sure the nameservers return
results quickly.
