This looks like a Squid bug with pipelined connections.

A workaround could be to limit the number of concurrent requests to 1 /
client connection.

Change
    if (nrequests >= 2) {
to
    if (nrequests >= 1) {
in src/client_side.c:clientReadRequest (line 2696 in my copy)
