replacing ACL-ed out URLs with image

squid-2.3.STABLE3-1:

i tried to replace "access denied" with a image such as
 deny_info http://localhost/whynot.gif acl_name
but it didn't work

even putting customized http page such as
 deny_info MyErrHeader acl_name

worked SOMETIMES (!) - mean sometimes MyErrHeader was displayed, sometimes
browsers (not squid's) error "Access denied"

Any ideas please?
