IP Masquerading is more 'efficient' than an application level proxy in
one sense (it requires almost no computing resources--while Squid is a
resource hog in comparison). But if efficiency also takes into account
bandwidth and latency, Squid is a much more efficient solution. Just
depends on your needs.

They are simply the ports that Squid will allow clients to connect to.
Nothing complicated about it, really. Squid has ACLs for everything. ;-)
